Procedures for Cook County Circuit Court Chancery Division

Filing – You must have Chancery Division cover sheet, check for fee or Petition for Indigency Status, Original Complaint, and Summons prepared to file. File in room 802 Daley Center or Suburban Branch of Court.

Forms – (Such as cover sheet and summons) available on line at web site for Clerk of Circuit Court of Cook County:
